What is Person Centred Planning?

Person Centred Planning is a set of approaches that help a person with a developmental disability to explore the things in life that are most important to them. Planning helps people, their family, friends and other members of their support network to focus on key areas of life identified. Planning services can be accessed through Developmental Services Ontario.

 

How Does Person Centred Planning Work?

Our trained and experienced planning facilitators assist people with developmental disabilities to create a plan in collaboration with their support network. This plan becomes the framework of their supports. A support plan helps people explore the things that are most important to them in their life today. The facilitator will guide the process and assist in the creation of meaningful and concrete goals which help to guide the person and their support network.

The development of a plan helps people to see a more positive future. The planning process helps people to develop goals and a plan on how they can be achieved. Person Centred Planning can also be extremely helpful to support professionals, families and other members of the support network to clearly understand the wants and needs of a person with a disability.

Community Living London planning facilitators are experienced in using a variety of tools to assist people with disabilities and their families to create a plan. At the end of the process, you will have a plan which will provide clarity, establish concrete goals and see a more positive, possible future.

Person Centred Planning links people with the services with which they would like to be connected. Services may include:

Life Planning

Staff work with adults to create a personalized Life Plan that identifies individualized strengths, interests and personal goals.

  • Community Access

    Linking adults and their families to supports and services already available in their community. Learn More

  • Transitional Planning

    Assist people to transition into new phases of their life. Our Planning Facilitators can help provide support and links to resources. Learn More

  • Planning Facilitation

    Provide adults and their families with ongoing support and links to community resources as they move toward achieving their goals. Learn More

  • Advocacy

    Advocate with or on behalf of people who have a developmental disability and are facing barriers in the community. Learn More
