What is Person Centred Planning?

We work with adults over the age of 18 with a developmental disability and their families to create an individual life plan that identifies each person’s support goals and dreams.

How Does Person Centred Planning Work?

Our experienced Person Centred Planning Facilitators work with each person and their support network to identify their own goals or dreams for the future. We facilitate the links to services and community based resources that match their goals and support requirements.

Personal plans of action are developed that assist each person to achieve his or her chosen goals in life.

What services are available?
