At the heart of all supports & services

Our person-centred planing facilitators assist individuals and families to develop a customized plan that identifies goals and makes links to community resources.
Resources may include supports and services provided directly by Community Living London, as well as those operated by other service providers.

How does the Planning Process work?

Central to the planning process are the Personal Outcome Measures. These Personal Outcome Measures form the basis for the planning process and ensure that supports are individualized to the individual’s preferences, priorities, and perspectives. The process can also include such planning tools as PATH and MAPS.

What is a Life Plan?

The Life Plan is a document that is developed over time in conjunction with a person and the people who know him/her well i.e. family, support staff, friends, etc. This group is more formally known as a Support Network.

It identifies the following:

  • Who the person is in terms of their strengths, gifts and talents
  • Dreams for the future
  • Identification of specific Goals and Personal Outcomes
  • History of significant milestones
  • Support Network members and friends

It is a living document that changes as the person changes and provides the framework for the design and delivery of supports. The Planning Facilitator oversees the ongoing development of the Life Plan.

What is a Planning Facilitator?

Someone who listens, acts as a guide and a link to community resources. The Planning Facilitator oversees the planning process. We meet at least once a year and more often if necessary.

What is a Support Network?

It is a group of people who know the person well, who come together to assist in the planning for a positive future. It may include family members, friends, teachers, support staff, and other members of the community.

The Planning Meeting

At least once a year, a person supported by Community Living London, and his/her support network will meet to discuss and outline their goals and Personal Outcomes for the coming year.

What can I expect at a planning meeting?

Planning meetings are an opportunity to discuss the person’s goals and the things that are important to them and to develop action plans to meet those goals. Topics of discussion may include: recreational and leisure pursuits, volunteerism, educational opportunities, living options, employment, family involvement, future planning, etc. This may also be a time to problem solve around a specific issue or to celebrate successes with the person. The Planning Facilitator will complete the minutes and identify the specific actions and the person responsible for following through on those actions. A future date is set to meet to review the Action Plan.
