What is f.y.i.?
f.y.i. (Facilitating Youth Independence) helps young adults who have an intellectual disability transition into community life after high school. Youth 18-22 are connected to community opportunities for leisure, practical learning and volunteering.
How does f.y.i. work?
Step 1: Planning - Youth begin f.y.i. by creating a Transitional Plan that identifies skills, interests and personal goals.
Step 2: Connecting & Supporting - Youth are connected to volunteer, practical learning and leisure activities in the community. Support is offered individually or in small group.
Step 3: Evaluation - Ongoing evaluation identifies achievements and progress toward independence.
Step 4: Exit Planning - The goal of f.y.i. is to help youth make long-term, natural connections to the community. Exit planning is the final step.
How do I get involved?
f.y.i. is for youth in London and Middlesex County who are seeking independence and are motivated to take an active role. Youth can self-refer or be referred by teachers, family or professionals.
